[US] AAP 2026 Infant Safe Sleep and Pediatric Visit Guidelines

American Academy of Pediatrics recommendations for room-sharing, back-to-sleep position, and well-child visit milestones in the US.

AAP 2026 Infant Safe Sleep and Well-Child Visit Guidelines: Evidence-Based Standards

The American Academy of Pediatrics (AAP) establishes rigorous clinical recommendations designed to eliminate sleep-related infant deaths, predominantly Sudden Infant Death Syndrome (SIDS) and accidental suffocation. Adhering to the ABCs of safe sleep and scheduled pediatric well-child checks ensures comprehensive infant protection.

1. The ABCs of Infant Safe Sleep

  • A - ALONE: The infant must sleep on an individual sleep surface. Never co-sleep on adult beds, couches, or armchairs due to severe entrapment and overlay hazards.
  • B - BACK: Always place infants on their backs (supine position) for every sleep until 12 months of age. Prone or side sleeping significantly increases airway obstruction risk.
  • C - CRIB: Use a firm, flat, non-inclined mattress covered solely by a tight-fitting sheet. Keep the crib completely free of soft objects, loose blankets, pillows, bumper pads, and stuffed toys.
  • Room-Sharing: Place the infant crib in the parents bedroom close to the parental bed for at least the first 6 months, reducing SIDS risk by up to 50%.

2. Recommended Well-Child Visit Cadence

Standard developmental screenings occur at: Newborn (3–5 days), 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 24 months, and 30 months.
