Safe Pediatric Antipyretic Usage: Weight-Based Paracetamol Dosage Guidelines
The golden clinical rule: 10-15 mg/kg per dose every 4-6 hours. Preventing hepatic toxicity and distinguishing teething from infectious fever.
1. Pathophysiological Understanding of Pediatric Febrile Responses
Fever is not an independent pathology, but a benign physiologic host defense mechanism driven by hypothalamic set-point elevation to curb pathogen replication and stimulate cellular immune response. Pharmacologic antipyretics are indicated primarily for relief of pain and severe discomfort, or when axillary temperature exceeds 38.5°C.
2. Weight-Based Paracetamol (Acetaminophen) Dosing Formula
- Standard Therapeutic Dose: 10 to 15 mg per kilogram of body weight per dose. Dosing must always be determined by the infant measured WEIGHT, not by generalized age categories.
- Dosing Frequency & Intervals: Administer every 4 to 6 hours as needed. Never exceed a cumulative 24-hour ceiling of 60 mg/kg to prevent irreversible hepatocyte necrosis.
- Volumetric Precision: Always utilize the calibrated oral measuring syringe or dropper supplied with the pediatric suspension. Household teaspoons vary widely and invite critical dosing errors.
3. Critical Antipyretic Contraindications
Never administer Aspirin (acetylsalicylic acid) to pediatric patients due to the catastrophic risk of Reye Syndrome (acute encephalopathy with hepatic fatty degeneration). Avoid alternating or combining Paracetamol and Ibuprofen without explicit pediatric instruction. Any infant under 3 months presenting with fever requires immediate emergency hospital evaluation rather than home antipyretic suppression.
